Please complete this form for each of your children who will be participating in youth programming during the Chagim. If you have more than 4 kids, please re-visit the page to complete a 2nd form.
* As stated in our Youth Policy, ALL YOUTH must either be in Shul with an adult (encouraged for children 1st grade and up) or with the supervised youth activities. We know that the day can be long for everyone, but it is our collective responsibility to keep our families safe and make sure all children are accounted for and supervised! To that end, please share that expectation with your children, and also visit the High Holiday Volunteers page to help out!
** Please note: children should bring their own lunch each day. (No peanuts or peanut butter.) ** Only snacks will be served.
